1. Purpose
To describe the preventive‑maintenance schedule, responsibilities, and replacement (“swap”) process for LTV 1200 ventilators rented from NJRA.
2. Equipment Covered
- Model: Vyaire™ LTV 1200 
- Serial numbers and individual runtime hours are recorded and tracked by NJRA. 
- Each unit bears a tamper‑evident tag displaying: - Date of last PM 
- Total accumulated hours at that PM 
 
3. Preventive‑Maintenance Schedule
Full preventive maintenance
Every 2 years or 10,000 runtime hours (whichever occurs first)
Compressor replacement
Every 30,000 runtime hours
4. Facility Responsibilities
- Track Hours – Record each ventilator’s runtime hours at least quarterly. 
- Notify NJRA When Due – Contact NJRA when 10,000‑hour threshold is reached. If after two (2) calendar years have elapsed since the last PM, NJRA will automatically ship a new unit. 
- Prepare Unit for Return – When a swap is arranged, package the outgoing ventilator in the shipping container provided and return it within five (5) business days of receiving the replacement. 
5. NJRA Responsibilities
- Maintain a master database of PM dates and runtime hours for each unit. 
- Automatic Swap: - If no facility notification is received by the two‑year anniversary of the last PM, NJRA will ship a fully serviced replacement unit automatically. 
 
- Facility‑Initiated Swap: - Upon receiving notice that PM is due, NJRA ships a replacement ventilator within three (3) business days. 
 
- Perform all inspections, calibrations, and compressor changes in accordance with manufacturer specifications and applicable regulations. 
6. Swap Procedure Summary
- Replacement Shipped – NJRA sends a serviced, patient‑ready unit. 
- Unit Changeover – Facility places the new ventilator into service. 
- Return Shipment – Facility returns the due unit using the provided packaging/label. 
- Confirmation – NJRA confirms receipt and updates its PM log.
